Responsibilities/Duties:

• Collaboratively work with therapy teams and Rehabilitation Department to deliver effective treatments.
• Double-check that all evaluations and necessary documentation have been completed.
• Stay up-to-date with changing technologies: Rapidly changing technologies are changing the way we communicate and assess. Keep up-to-date with the latest tools, trends and applications in the market to stay ahead.
• Offer therapy services that conform to the recommendations detailed in the treatment plan and any relevant prescriptions.
• Furnish therapies that aid in refining patients' skills and/or challenges.
• Expertly use electronic record keeping systems to ensure that treatment documentation is accurate and up-to-date.
• PDPM (Patient-Driven Payment Model) requires healthcare providers to accurately document patient needs and conditions to ensure appropriate reimbursement.
• Ensure that therapy treatments and plans are accurately documented and reported in line with the Medicaid and Medicare billing requirements.

Qualifications:

• The issuance of a New Jersey PT license signifies the competence and skill of the Physiotherapist to provide quality care to patients.
• With years of experience in long-term care, specifically in Skilled Nursing Facilities, I have had the opportunity to gain the knowledge and expertise necessary to deliver the best possible care to residents.
• Skillfulness in communication, interpersonal relations, and organizational capabilities.
• Being skilled at managing patients with diverse personalities and abilities is an indispensable talent that healthcare providers must possess to provide excellent care.
• Creative problem-solving skills.
