Responsibilities/Duties:

• Interact with therapy teams and Rehabilitation Department to provide holistic care.
• Guarantee that all assessments and essential paperwork have been concluded.
• Foster a culture of open communication: Encouraging an open and honest culture of communication will promote trust and transparency, making it easier to share feedback and ideas.
• Administer therapy according to the treatment plan and any prescribed medications to ensure optimal results.
• Give remedies that assist in developing patients' abilities and/or disabilities.
• Expertly use electronic record keeping systems to manage treatment documentation, and track patient progress.
• Healthcare organizations should train staff and implement necessary changes to effectively operate under the PDPM (Patient-Driven Payment Model) system.
• Design therapy treatments and plans that are consistent with the Medicaid and Medicare billing procedures.

Qualifications:

• Meeting continuing education requirements is a must for physical therapists in New Jersey to maintain the validity of their PT license.
• I am a seasoned professional with extensive experience in long-term care, specifically in Skilled Nursing Facilities, where I have been dedicated to providing top-notch healthcare services and ensuring the well-being of residents.
• Impressive interpersonal, communication, and organizational abilities.
• The capability to manage diverse patients with varying personalities and abilities is a critical ability that healthcare practitioners must possess.
• Creative problem-solving skills.
